Those "vague rumors" should have been investigated at the time by management.
Ed Joe thought that Stevens had applied some fixes, three years ago. Yet the forensic report indicates that an even greater variety of foibles/deficiencies occurring these days. These factors add strength to reasoning for why its important to adopt ongoing reviews. Prevention of back sliding is a necessary remedy. Thus, adding an internal auditor is a demonstrated need.
