The Facilities Manager works directly for the GM.  The GM would be the person to notify.

Of course your premise is off base. The GM already knew, as these reports generally go to the GM, not necessarily to department heads or even the board, or even more rarely to the facilities manager All reports from committees even go through the GM to the board. 

But heck. Looks like someone wants to crucify the guy regardless (or anyone but the GM who is the responsible party) and make comments as though he did something wrong while covering with the "IF" word.
